The positive support from the Form Team. The support team with Form is great. From the initial setup person to my personal team of physician, physician assistant, nutritionist, and general support has been great. Always professional, kind, and understanding of my personal health and weight challenges. They do not make me feel like just a patient of Form. Instead, I feel like this program is specialized to help me be successful in reaching my goals in the healthiest way for me.

Worth the wait. Yes, the wait time was longer than I expected, but well worth the wait. Completed my first appointments with a Doctor and a Nutritionist. Both appointments went great and I learned a great deal. We set goals and I was provided with great resources. Looking forward to starting this journal. Feeling highly confident in this process.

Would put negative star if possible Would put negative star if possible. Signed up for their service on 2/15. Told there system had high volume and I would get to talk to a doctor in March?!?! That information would have been nice to know before I spent an hour filing in my medical history. I was able to go with another provider site and get a doctor the SAME day. Also the doctor emailed follow help, had great bedside manner and is local. Total night and day experience comparison.
