Terrible experience Terrible experience. Paid $99 and I was totally unable to set an appointment or communicate with Form. Then I all of a sudden was able to set up appointments only to have them both cancelled on me days before they were to happen. I have tried to reach out in all ways only to get no response. I feel like they have taken my money and provided absolutely nothing in return.

Finally, a REAL solution! Having done literally every program out there (seriously—name one! Done it) over the last 30+ years, I’m thrilled to have finally found something that works AND is sustainable. My favorite thing is the support I feel, from the admin staff, to the medical team and the nutritional support. I’ve never felt so confident that something is going to last and not just be a short term fix. I’ve been a FORM member for 9 months, which is honestly the longest I’ve stuck with anything. That’s because it’s easy! Sure, I have to put in the effort, but they make it feel easy. I still wake up every day and can’t believe it’s real.

The app is extremely easy to use to… The app is extremely easy to use to track weight, schedule and attend appointments and staff is very timely in responding to messages. The combination of dietician plus medical providers is very effective. The classes are very informative and helpful. I highly recommend Form!

The Program The Doctors, Dieticians, accountability with pix of food and scale weight going directly to Form are all wonderful aspects. I’ve lost 43# in a year & 10 # before I recieved my scale! I am off my prediabetic med! A1C 5.1 now! I am off one of my b/p meds as well! My Doctor is so sweet and has a great bedside manner! She is informed and very thorough! My dietician is wonderful and gives great advice when I have a challenge diet related problem!
